What is Tryptyr?
Tryptyr is used for dry eye disease (DED) and helps your eyes to produce more natural tears. It is a prescription eye drop administered twice a day (approximately every 12 hours).
Tryptyr is thought to work by activating trigeminal nerve pathways to stimulate natural tear production. It belongs to the drug class called TRPM8 thermoreceptor agonists and was the first of its class to be approved.
Tryptyr (acoltremon ophthalmic solution) 0.003% gained FDA approval on May 28, 2025.
- This approval was based on the results of 2 clinical trials (COMET-2/3) with 930+ dry eye patients.
- Significantly more patients achieved at least a 10mm increase in natural tear production at day 14 compared to those using a placebo eye drop: 42.6% vs 8.2% (COMET-2) and 53.2% vs 14.4% (COMET-3).
- It took one day for Tryptyr to start working.
Side effects
The most common side effect of Tryptyr is eye pain after putting in the eye drops.
Serious side effects and warnings
Potential for Eye Injury and Contamination. To avoid the potential for eye injury and contamination, do not touch the vial tip to the eye or other surfaces.
Use with Contact Lenses. Tryptyr should not be administered while wearing contact lenses. If contact lenses are worn, they should be removed before administration of the solution. Lenses may be reinserted 15 minutes following administration of Tryptyr.
Use with Other Ophthalmic Drugs. If more than one topical ophthalmic drug is being used, the drugs should be administered at least five (5) minutes apart.

Before using
Before using Tryptyr, tell your healthcare provider about all your medical conditions, such as if you:
- Have any allergies to acoltremon, Tryptyr, or any of the ingredients in the eye drops
- Wear contact lenses
- Have any problems with your eyes, in addition to DED
- Are pregnant or intend to become pregnant
- Are breastfeeding or intend to breastfeed.
Pregnancy
There are no studies on the use of Tryptyr during pregnancy. Talk to your healthcare provider about the best way to treat your DED if you are pregnant.
Breastfeeding
It is not known if Tryptyr passes into breastmilk. Talk to your doctor about the best way to feed your baby during this time.
How do I use Tryptyr?
Use Tryptyr exactly as your healthcare provider tells you to. It is for use in the eye. Each single-dose vial of Tryptyr will give you enough medicine to treat both your eyes.
- There is extra Tryptyr in each single-dose vial in case you miss getting a drop into your eye.
Keep the unopened single-dose vials in the original foil package until ready to use.
- Wash your hands before each use to make sure you do not infect your eyes.
If you use Tryptyr with other eye medicines, wait at least 5 minutes between using Tryptyr and the other medicines.
If you wear contact lenses, remove them before using Tryptyr. Wait 15 minutes after dosing before putting contact lenses back in your eyes.
- Use 1 drop of Tryptyr in each eye 2 times each day.
- Use 1 single-dose vial in the morning and another single-dose vial in the evening, approximately 12 hours apart.
Do not let the tip of the single-dose vial touch your eye or any other surface to avoid eye injury or infection.
- Do not use the single-dose vial or the medicine inside it if the tip touches your eye or another surface. Throw it away.
Tryptyr vial is for 1-time use only. Use right away after opening and do not reuse.
Call your healthcare provider right away if you get an allergic reaction or other eye problems such as eye injury, eye infection, or eye pain. For additional safety information, see the Full Prescribing Information and talk with your healthcare provider.
Dosing information
Dose of Tryptyr for dry eye disease in adults
- 1 drop twice daily into each eye 2 times daily.
What happens if I miss a dose?
If 1 dose is missed, treatment should continue at the next scheduled time.
Storage
Store Tryptyr in the unopened carton.
- After opening the carton, Tryptyr may be kept refrigerated or at room temperature at 36°F to 77°F (2°C to 25°C).
- If stored at room temperature, use Tryptyr within 30 days. Throw away (dispose of) unused Tryptyr after 30 days if stored at room temperature.
- Throw away (dispose of) unused single-dose vials 7 days after opening the foil pouch.
Always check the expiration date on the vial before use. Do not store after the expiration date. Throw away (dispose of) Tryptyr after the expiration date on the carton and pouch.
Do not leave Tryptyr in a car, outdoors, in the sunlight, or in any other place where temperatures can go above room temperature [greater than 77°F (25°C)].
Keep out of reach of children.
Ingredients
Active: acoltremon 0.003%
Inactive ingredients: polyoxyl 35 castor oil, sodium dihydrogen phosphate dihydrate, sodium chloride, hypromellose, and purified water (inactive ingredients).
Sodium hydroxide adjusts pH. No antimicrobial preservative is included.
Supplied as a sterile, clear to slightly opalescent, and colorless solution in a low-density polyethylene (LDPE), single-dose vial with a 0.4 mL fill.
- One strip of 5 single-dose vials is packaged in a foil pouch with twelve (12) pouches in a carton.
