Important warnings
This medicine can cause some serious health issues
You should not use estradiol if you have: undiagnosed vaginal bleeding, liver disease, a bleeding disorder, if you will have major surgery, or if you have ever had a heart attack, a stroke, a blood clot, or cancer of the breast, uterus/cervix, or vagina.
Do not use if you are pregnant.
Estradiol may increase your risk of developing a condition that may lead to uterine cancer. Report any unusual vaginal bleeding right away.
Using this medicine can increase your risk of blood clots, stroke, heart attack, or cancer of the breast, uterus, or ovaries. Estradiol should not be used to prevent heart disease, stroke, or dementia.
Get emergency medical help if you have any of these signs of an allergic reaction while taking estradiol topical (the active ingredient contained in Estring) hives; difficulty breathing; swelling of your face, lips, tongue, or throat.
Remove the vaginal ring and seek emergency medical attention if you have: fever with nausea, vomiting, diarrhea, muscle pain, dizziness, fainting, and/or sunburn-like skin rash. These may be signs of a life-threatening disease called toxic shock syndrome.
Call your doctor at once if you have:
-
heart attack symptoms--chest pain or pressure, pain spreading to your jaw or shoulder, nausea, sweating;
-
signs of a stroke--sudden numbness or weakness (especially on one side of the body), sudden severe headache, slurred speech, problems with vision or balance;
-
signs of a blood clot--sudden vision loss, stabbing chest pain, feeling short of breath, coughing up blood, pain or warmth in one or both legs;
-
swelling or tenderness in your stomach;
-
jaundice (yellowing of the skin or eyes);
-
memory problems, confusion, unusual behavior;
-
unusual vaginal bleeding, pelvic pain;
-
a lump in your breast; or
-
high levels of calcium in your blood--nausea, vomiting, constipation, increased thirst or urination, muscle weakness, bone pain, lack of energy.
Common side effects may include:
-
nausea, vomiting, bloating, stomach cramps;
-
headache;
-
swelling in your hands or feet, weight gain;
-
breast pain; or
-
vaginal itching or discharge, changes in your menstrual periods, breakthrough bleeding.

General adverse events
The most commonly reported side effects were headache, upper respiratory tract infection, moniliasis, genital pruritus, vulvovaginal mycotic infection, and diarrhea.[Ref]
Cardiovascular
- Common (1% to 10%): Hot flushes
- Frequency not reported: Deep and superficial venous thrombosis, thrombophlebitis, myocardial infarction, stroke, increase in blood pressure, congestive heart disease
- Postmarketing reports: Palpitations, ventricular extrasystoles, chest discomfort, increased heart rate[Ref]
Oncologic

Hypersensitivity

- Postmarketing reports: Toxic shock syndrome (fever, nausea, vomiting, diarrhea, muscle pain, dizziness, faintness, or a sunburn-rash on face and body)[Ref]

- Common (1% to 10%): Urinary tract infection, leukorrhea, vaginitis, vaginal discomfort/pain, vaginal hemorrhage, asymptomatic genital bacterial growth, genital moniliasis, vulvovaginal mycotic infection, vulvovaginal pruritus, bladder discomfort
- Frequency not reported: Changes in vaginal bleeding pattern and abnormal withdrawal bleeding or flow, breakthrough bleeding, spotting, dysmenorrhea, increase in size of uterine leiomyomata, vaginal candidiasis, change in amount of cervical secretion, changes in cervical ectropion, application site reactions of vulvovaginal discomfort including burning and irritation, genital pruritus, endometrial hyperplasia
