What is Ekterly?
Ekterly is a prescription medication used to treat acute attacks of hereditary angioedema (HAE) in people 12 years of age and older. Ekterly tablets are taken as early as possible, wherever they are, to treat painful, debilitating, and sometimes life-threatening HAE attacks.
Ekterly is available as an oral tablet, which is an advantage over other treatments that require intravenous or subcutaneous administration and may need to be administered in a healthcare facility.
Ekterly (sebetralstat) is a novel plasma kallikrein inhibitor.
Ekterly works by blocking kallikrein, an enzyme that plays a crucial role in dilating blood vessels and increasing fluid leaking from capillaries. Blocking kallikrein reduces levels of bradykinin, the peptide that causes swelling during HAE attacks.
Ekterly side effects
The most common Ekterly side effect is headache, which affects 3% of patients.

Before taking this medicine
Tell your healthcare provider about all of your medical conditions, including if you are:

Pregnant or planning to become pregnant, as it is not known if this medicine can harm your unborn baby.

Breastfeeding or planning to breastfeed, as it is not known if this medicine passes into your breastmilk. Talk to your healthcare provider about the best way to feed your baby while taking this medicine.

Have liver problems.
How should I take Ekterly?
Usual Ekterly dose
Ekterly tablet strength: 300mg.
Take 2 tablets (600mg) by mouth at the earliest sign of an acute attack of hereditary angioedema.
If you do not feel better or if your symptoms worsen or come back, take another 2 tablets (600mg) at least 3 hours after the first dose.
Dose modifications
If you take certain medicines or if you have liver problems, your healthcare provider may change your dose to 1 tablet (300mg), by mouth, at the earliest sign of an acute attack of hereditary angioedema. If you do not feel better or if your symptoms worsen or come back, take another 1 tablet (300mg) at least 3 hours after the first dose.
Take Ekterly exactly as your healthcare provider tells you to take it.
What happens if I overdose?
If you take too much of this medicine call your healthcare provider or Poison Help line at 1-800-222 1222 or go to the nearest hospital emergency room right away.
Interactions
Tell your healthcare provider about all of the medicines you take, including prescription and over-the-counter medicines, vitamins, and herbal supplements. Taking Ekterly with certain other medicines can cause side effects or affect how well Ekterly or the other medicines work. Ekterly has a drug interaction with moderate and strong CYP3A4 Inhibitors, and moderate or strong CYP3A4 inducers. You may not be able to take Ekterly with these medications, or your dose may need to be reduced.
It is important to tell your healthcare provider if you take any of the following, as their use with Ekterly is not recommended:
- Itraconazole
- Phenytoin
- efavirenz
