Enalapril / Felodipine Dosage

Usual Adult Dose for Hypertension
Initial dose: 1 tablet (enalapril-felodipine 5 mg/2.5 mg) once a day. Determine dosage by individual titration.
If blood pressure control is inadequate after one to two weeks, the dose may be increased to 2 tablets (enalapril-felodipine 5 mg/5 mg) administered once daily. The next incremental effect can be achieved with 4 tablets (enalapril-felodipine 5 mg/2.5 mg) administered once daily. If control remains unsatisfactory, the addition of a thiazide diuretic should be considered.
Renal Dose Adjustments
CrCl < 30 mL/min:
Oral: The initial dose of enalapril is 2.5 mg once a day, titrated upward until blood pressure is controlled.
Liver Dose Adjustments
In patients with impaired liver function, a starting dose of felodipine 2.5 mg orally once a day is recommended. Daily doses generally should not exceed 10 mg.
Dose Adjustments
The dose may be increased after one or two weeks and should be adjusted to blood pressure response.
Precautions
ACE inhibitors can cause injury and death to the developing fetus in the second and third trimesters of pregnancy.
Dialysis
Enalapril is removed by hemodialysis (20% to 50%).
Hemodialysis: Enalapril 2.5 mg on dialysis days. The dosage on non-dialysis days should be adjusted according to blood pressure response.
