What is the best way to stop taking Protonix after using for 3 years?

I have been on Protonix for 3 years and read it is not good to just stop taking this drug. What is the safest way to get off of Protonix after this length of time?

Medications taken together?

Can I take Carafate and Protonix at the same time? I take Protonix 40mg twice daily and Carafate four times daily. Do I have to wait between taking these two medications?

I take pantoprazole (protonix) and I have had a sore throat/hoarseness for a few months?
