The Zepbound Coupon or Savings Card helps you save money on your prescription for Zepbound.
You may pay as little as $25 for a 1-month to 3-month prescription of Zepbound if you qualify for the Zepbound Savings card, depending on your insurance status.
How do I qualify for a Zepbound Coupon or Savings Card?
You can get a Zepbound savings card if you have commercial medical insurance and have a Zepbound prescription for weight loss or obstructive sleep apnea that matches with the FDA-approved product labeling. You must be a current resident of the United States or Puerto Rico and be 18 years old. You are not eligible for a savings card if you do not have commercial drug insurance or if you are enrolled in any state, federal, or government-funded healthcare program, including, without limitation, Medicaid, Medicare, Medicare Part D, Medicare Advantage, Medigap, DoD, VA, TRICARE CHAMPUS, or any state prescription drug assistance program.
The savings card can be used with the single-dose pen in 2.5 mg, 5 mg, 7.5 mg, 10 mg, 12.5 mg, and 15 mg strengths.
How much will I save with a Zepbound Coupon or Savings Card?
If you have commercial insurance that covers Zepbound, you may pay as little as $25 for 1, 2, or 3 months of medicine, up to a maximum savings of $150 per month, $300 for two months, $450 for three months and a separate maximum annual savings of up to $1,950 per calendar year. You must have a valid prescription for Zepbound that has been prescribed for the FDA-approved indication of type 2 diabetes in adults. The Zepbound card may be used for a maximum of up to 13 prescription fills per calendar year. Participation in the Program requires a valid patient HIPAA authorization.
If you have commercial insurance that does not cover Zepbound, you may have savings of up to $469 off your 1-month prescription fill of Zepbound. There is a maximum monthly savings of up to $469 and a separate maximum annual savings of up to $3,283 per calendar year. The card may be used for a maximum of up to 7 prescription fills per calendar year. You must have a valid prescription for Zepbound that has been prescribed for FDA-approved indications. To use this savings plan requires a valid patient HIPAA authorization. A month is defined as 28-days and up to 4 pens.

How do I apply for the Zepbound Coupon or Savings Card?
You can apply for the Zepbound Savings Card online; there is a link on the Zepbound Price Guide page. Fill out the enrollment form, and if you are entitled to it, you will receive your activated Savings Card by downloading it, emailing it to your address, or adding it to your mobile wallet. You may need to get prior authorization (PA) from your doctor before insurance will cover your medicine, this is a form that your doctor will fill out. Show the card at the pharmacy when you fill your prescription. You may also get Zepbound delivered to your home through Lilly Direct, and they will automatically apply your savings.
