Drug Interactions between fosphenytoin and posaconazole
This report displays the potential drug interactions for the following 2 drugs:
- fosphenytoin
- posaconazole
Interactions between your drugs
fosphenytoin posaconazole
Applies to: fosphenytoin and posaconazole
Consumer information for this interaction is not currently available.
GENERALLY AVOID: Phenytoin and other enzyme-inducing anticonvulsants may decrease the plasma concentration of posaconazole. The mechanism is induction of posaconazole clearance via the UDP-glucuronosyltransferase glucuronidation and P-glycoprotein efflux pathways. According to product labeling, coadministration of posaconazole (200 mg tablets once a day) with phenytoin (200 mg once a day) for 10 days resulted in reduced posaconazole mean peak plasma concentration (Cmax) by 41% and systemic exposure (AUC) by 50%. Treatment failure could result. Conversely, posaconazole is an inhibitor of CYP450 3A4 and can increase the plasma levels of phenytoin, which is thought to be partially metabolized by the isoenzyme. Phenytoin Cmax and AUC increased by 16% during coadministration of posaconazole.
MANAGEMENT: Use of posaconazole in combination with enzyme-inducing anticonvulsants should generally be avoided unless the potential benefit outweighs the risk. If coadministration is considered necessary, patient response to the antifungal should be closely monitored. In addition, pharmacologic response and serum phenytoin levels should be closely monitored following initiation or discontinuation of posaconazole, and the phenytoin dosage adjusted, as necessary.
Drug and food interactions
posaconazole food
Applies to: posaconazole
Food significantly increases the absorption of posaconazole from the tablet or suspension formulations. The manufacturer recommends that you take the tablet with food and the oral suspension during or immediately (within 20 minutes) after a full meal. If you cannot eat a full meal, you should take the oral suspension with a liquid nutritional supplement like Ensure or an acidic carbonated beverage like ginger ale. Do not take these medications on an empty stomach, as it may lead to inadequate blood levels and reduced effectiveness. Ask your doctor before using alcohol together with posaconazole from delayed release suspension formulations as this may cause an increase in side effects. Talk to your doctor or pharmacist if you have any questions on how to use the medication properly.
Therapeutic duplication warnings
No warnings were found for your selected drugs.
Therapeutic duplication warnings are only returned when drugs within the same group exceed the recommended therapeutic duplication maximum.
