Usual Adult Dose for CMV Retinitis
330 mcg (0.05 mL) by intravitreal injection into the affected eye for both induction and maintenance. The induction phase consists of one injection every other week for two doses. Thereafter, maintenance doses should be administered once every four weeks.
Disease progression that occurs during maintenance therapy may require a reinstitution of induction therapy to restore disease control.

Precautions
Ocular inflammation (uveitis) occurs in approximately 25% of patients, particularly during induction dosing. Topical corticosteroids may be necessary to treat the inflammation so that fomivirsen therapy may continue. Once the CMV retinitis is under control, an interruption of therapy may be worthwhile if the patient has an unacceptable level of inflammation.

Fomivirsen intravitreal injection is a localized treatment for CMV retinitis. Fomivirsen therapy does not treat or prevent systemic and extraocular CMV disease or disease in the untreated eye.
Treatment with fomivirsen intravitreal injection is not recommended for patients who have recently (within the last 2 to 4 weeks) been treated with either intravenous or intravitreal cidofovir because of the risk of excessive ocular inflammation.
