Diltiazem / Enalapril Dosage

Usual Adult Dose for Hypertension
Initial dose: 1 tablet (diltiazem 180 mg/enalapril 5 mg) orally once a day.
Maintenance dose: 1 to 2 tablets (diltiazem 180 mg/enalapril 5 mg) orally in 1 to 2 divided doses.
Renal Dose Adjustments
Because enalapril is primarily eliminated by the kidneys, higher doses are not recommended in patients with renal disease.
Liver Dose Adjustments
Because diltiazem is primarily eliminated by the liver, higher doses are not recommended in patients with liver disease.
Dialysis
Diltiazem does not appear to be removed by peritoneal or hemodialysis.
Enalapril is removed by hemodialysis (20% to 50%).
Other Comments
It is usually appropriate to begin therapy with diltiazem-enalapril only after a patient has failed to achieve the desired antihypertensive effect with either drug alone at maximally recommended doses. It may also be used if the dose of one drug by itself cannot be increased further because of dose-limiting side effects.
