Usual Pediatric Dose for Botulism
Under 1 year old: 1 mL/kg (50 mg/kg) IV
- Begin infusion slowly, at 0.5 mL/kg/hour (25 mg/kg/hour); if no untoward reactions after 15 minutes, may increase rate to 1 mL/kg/hour (50 mg/kg/hour) - DO NOT EXCEED THIS RATE OF ADMINISTRATION
- Monitor the patient closely after each rate change.
Use(s): Treatment of infant botulism caused by toxin type A or B in patients below 1 year of age.

Administration advice:
- Do not pre-dilute before infusion.
- Begin infusion within 2 hours after reconstitution is complete, and conclude within 4 hours of reconstitution, unless infusion is temporarily interrupted for an adverse reaction.
- Administer using low volume tubing and a constant infusion pump through a separate infusion line.
- If a separate line is not possible, may piggyback into a pre-existing line containing either sodium chloride injection USP or one of the following dextrose solutions (with or without sodium chloride added): 2.5, 5, 10, or 20% dextrose in water.
- If a pre-existing line is used, do not dilute to more than 1:2 with any of the above-named solutions.
- Use an in-line or syringe-tip sterile, disposable 18 micrometer filter for administration.
- Do not exceed the recommended dose, concentration and rate of infusion.
- Begin infusion slowly, at 0.5 mL/kg/hour (25 mg/kg/hour); if no untoward reactions after 15 minutes, may increase rate to 1 mL/kg/hour (50 mg/kg/hour) - DO NOT EXCEED THIS RATE OF ADMINISTRATION
Reconstitution/preparation techniques:
- This product is preservative free; after reconstitution, enter only once for the purpose of administration.
- Reconstitute with 2 mL sterile water for injection (to make a 50 mg/mL solution).
- Rotate container gently to wet all the powder; allow approximately 30 minutes for all the powder to dissolve.
- DO NOT SHAKE VIAL; foaming will occur.
