Hi, Bird!
There is no set dosage because it's dosed according to the condition being treated, the severity, of the patient's symptoms, the patient's age and history. Everybody will respond differently to this type of medication and your individual response will determine your optimum dose.

I'm not sure I'm clear about what you're asking about the two medications. Did you mean taking them in combination?

The combination presents a slight risk of a potentially serious but rare interaction. Doctors are aware of this risk but for most people the benefits far outweigh the risk.
How long have you been taking the two medications? Although improvements may appear early in treatment it can take 4-8 weeks for the drugs to reach maximum therapeutic levels.
Talk with your doctor. Maybe giving it a bit more time and see how you're doing. See what the doc thinks about increasing your dose. The dosages you plan are well within the recommendations.
