Who recommended that you wean off Xarelto? If it was your doctor, then they should have told you how they were going to do it. They are the only one that can write prescriptions as the weaning process is completed. If your doctor is not willing to share the plan, then I would seek out a Hemotologist to evaluate your situation and come up with a plan.
