I started taking escitalopram 20 mg without gradually increasing the dose from 10mg to 20mg. Is it harmful or can cause any serious health issues?

The starting dose is 10 mgs. in order to see how this antidepressant reacts, adjusts to your organism and how you respond to the medication , minimize potential side effects and risks.
Everyone reacts differently to medications and you may or may not experience potential side effects, starting at such high dose increases the chances of having potential side effects, some people experience some and some don't, as I mentioned it is different for everyone.
I suggest you discuss this further with the Dr. who prescribed this dose, try to restart with10 mgs., you do not mention how long you have been on this dose, it is important because if you have been on it for a while you may have withdrawal symptoms if you reduce the dose by half.
