I took Spiriva a couple of years ago for 6 weeks and gained an unsightly 22 lbs, all on the midsection and nowhere else. After dieting off and on for the last 6 months I still cannot lose the weight. My midsection is all bulged out and I look terrible. Does anyone have an answer to my problem. Bill

I wes prescribed Spiriva for COPD a little over two months ago. I'm a 71 year old man. My weight has held steady between 147- 150 for decades. Suddenly, with no change in diet, I found myself topping 155. Then 160. I have cut back my food intake to an oatmeal breakfast, an apple for "lunch", and one small serving at dinner. Still the weight creeps up, and I am experiencing the ugly bulging stomach that many here have already described. I have used only two of the twist top dispensers. No more. The spray was not all that effective anyway. I was still having to hit the rescue inhaler several times a day. It's not like I'm in the dating market at my age, but I've had a flat stomach all my life, and now I look awful. Reading these reports is discouraging. Anyone have long term follow up?
This is such a interesting conversation. I began using Spiriva very recently for COPD. Since using it, I noticed my belly extending in a balloon-like shape. None of my old clothing fits now. My weight has increased but not by much... 2 pounds or so. But based on how many others have experienced the body changes, I am now very tempted to stop the inhaler. I'm quite disgusted and unhappy about this as I'm already body-conscious and will work diligently (even with the breathing issues) to lose whatever I can to regain a body I can live with. I'm really sorry for others who have also experienced this; it's a side effect that should require further research.
I started using the Spiriva capsules with the handi inhaler in 2013. I have had stomach issues with digestion, reflux and IBS since the removal of my gallbladder in 1985. Other than occasional bloat and inflammation I have also had trouble gaining and maintaining my weight but managed to keep it at 52 to 56 kilos. 2016 with the COPD my weight dropped down to 42 kilos and it was close to the end of 2017 before I had gained back12 kilos. In November 2019 the application from handi inhaler to the Respimat was made. From the first application, my stomach extended and grew rapidly that I was looking like I was malnutrition like those kids in a third world country with a huge stomach, skinny arms and legs and my breathing was severely impacted. My diet or eating habits hadn't changed, I couldnt understand what I had done that was different until I remembered I was now taking the Spiriva Respimat.
It had felt like I had a rubber band around my middle stretched to its max, very tight, very uncomfortable and no give at all. So I hung in there waiting to see if my circumstance would change and to give the med time but 10 weeks later after gaining more than 20kilos (something I hadn't achieved in 35 years) I stopped using the Respimat and went back to the handi inhaler. Almost immediately I felt better the tight bloating went away along with the pressure off the lungs. It is now September and I have managed to lose about 6 kilos but still have the big stomach which will take longer I guess to go down. Keeping in mind I still use the Spiriva in the powder form but In the 6 months since stopping the spray mist form, I feel so much better, I still get the inflammation and bloat but nowhere to the extent of 24/7, 7 days a week that I was for those 10 weeks. Coming here and reading everyone's comments and with my own experience, has confirmed in my mind that this side effect exists, is real and not as rare as I initially thought.
I’m told SPIRIVA does not cause weight gain. I beg to differ!!! Not so much pounds, but my waistline and stomach blew up. I liked the medication until I noticed this. I have always had a small waist and relatively flat stomach. I look 5 months pregnant!!
How crazy is this, I have been researching why I am so bloated, looking 7 months pregnant, and so uncomfortable for months now with the answers not applying to me and my lifestyle. It never occurred to me it could be the Spiriva! I recently was prescribed an antibiotic and was researching if I could use my Spiriva inhaler while taking this antibiotic and came across this forum. I have been using the spiriva respimat inhaler for 7 months and have gained 27 pounds, and like everyone else it is all in my waistline! So uncomfortable! I am 64 years old, small framed and have never weighed more than 110 lbs. in my life other than when I was with child and even then the most I ever weighed at 9 months was 132 lbs. Very disheartening to hear everyone is having trouble getting back to their normal weight.
I'm wondering whether anyone who experienced weight gain had recently stopped smoking before taking Spiriva? My primary care doctor says the weight gain is from stopping smoking, your taste buds being able to taste food again, and you eating more as a result. Thoughts?
I was on spiriva for 3 years. I gained significant weight. I stopped the medication May 1st 2018. June 1 2018 i am down 17lbs. And have not changed anything else. I would say weight gain is a side effect... they were considering gastric bipass on me. I was 324 lbs (normally 250lbs) i am now 307 and dropping. I am on a stack of steroids for my lungs, i just chose the one i notice less effects from.
If anyone ever finds out why some are gaining weight with Spiriva, please report it here.
I gained 23 pounds in 14 months, mostly in my abdomen, but also in breasts and upper arms.
I started out at 118 lbs and when I got to 141, I quit. It is just like being pregnant. I only gained 20 pounds when I was pregnant 3 times. Otherwise, I have never weighed above 125.
My feet, knees, hips and back are having trouble carrying all this weight around.
If you have this problem, report it to the FDA. Go to FDA medwatch and make a report.
I took it for a brief time and gained 8 pounds in a week. I am on a very retricted diet so I know it was not increased eating. I have not been able to get rid of those pounds - no matter what I tried. I also notice lately that I seem to have more trouble with swollen ankles. Just had dental surgery and am on a diet of soft food only. After losing the water weight gained because of the surgery I notice that I am finally almost down to my pre-Spiriva weight. Yes - it could be because of the soft diet. My diet was restricted before because I had discovered I am allergic to all grains. Cutting out grains certainly should keep my weight stable - but no go. Anyway... while on this soft diet I notice that I am urninating very frequently and the swelling in my ankles has all but disappeared. I am wondering if the spiriva caused water retention. Anyway - fingers crossed. I gained 5 pounds with the dental surgery and have now lost 8 pounds.
That makes three pounds of the eight that I had not been able to get off before. Hoping to lose the additional 5 spiriva pounds soon. In any case - going on a soft food diet might be worth a try. I have been eating soup, sweet potatoes and ice cream and yogurt. Have also managed to eat pulled pork and finely shredded chicken. It is not too bad. All will be worth it if I can get both my teeth and my weight back to normal!
I started using spiriva about 6 months ago... the weight gain is really bothering me and like most here it is around the middle and protruding stomach. It is really worrying me i have never had a weight problem,slways been 52/55 kilos. I intend to go off it,see my doctor and either get some other medication if there is some avsilsble,but this weight gain is seriously affecting me. I feel so uncomfortable with the added weight.
I too have gained 26lbs right in the gut, (gezzz) I have even gotten breasts now.
I have been on Spiriva for 1 year now but noticed this gain within 5-6 months and Doc said that no way, "Well" it happened with my same diet.
I wonder exactly what's in the ingredients?

I had contact with a medical professional but along the line here but somehow lost him. My problem of weight gain from Spiriva still plagues me after two years of discontinuing it.
Every single pound is on the midsection only, and nowhere else. There have been others online that said the same thing: "All on the midsection and nowhere else." Just by that statement alone tells me that they truly have suffered the same side effect as I have. Along with that is this distended abdomen problem that is quite embarrassing. Even though it has been over two years since I last used the inhaler I have not been able to get rid of the weight. I do have a feeling that it very well could be damage to my system somehow. This is certainly a serious side effect, a dirty one, that should warrant that this damn thing be pulled from the market. Bill.

Ask your doctor to check you out for Cushing's Disease. Accumulation of fat on the abdomen but not on the limbs is one of it's symptoms.
I'm not going to make your day. Weight gain is not a side effect of spiriva. Particularly with using it for only 6 weeks. I may be able to give you an answer to the weight gain if you tell me are you male or female and your age.
Oh and are you on any medication now.
I am a 65 year old male and the weight gain is certainly a side effect. I picked up several comments from other people online who suffered the same reaction. I know they're not lying because every one of them said it was all on their midsection and nowhere else, and they cannot lose it. One woman commented that her 62 years old father took it for two years and gained 90 lbs!
This side effect is not benign. Someone said that he worked in fashion design and lost his job because of troubling weight after using it for three (3) months.

Spiriva weight gain problem: The only medication I am on is the antidressant Wellbutrin (Bupropian). This medication alone causes weight loss which it did when I first took it. It has had no bearing on the weight gained from Spiriva. Aside from this I have never, ever had a weight problem. I had been in a size 32 trousers since high school. I now seem to have a permanent 36 inch waist line bulge. I know this is a rare, and difficult problem to solve and perhaps there is none. No doctor has been able to help me. That's why I've come here to this open forum. Thank you, Bill.
As men get older, they tend to gain weight there because their testosterone level drops, that is why I suggested you have one done.
I've had my testosterone checked 4 times since then with the last by a urologist
3 months ago. You have to realize, as with the other people, that this is not a normal weight gain. Along with the weight gain is swelling inside of the belly. At times when I inhale, take in air, the area will suddenly bulge out causing a great deal of discomfort. I had a gastrointerologist examine me but could find nothing wrong other than stating that the bulge was unusal. I am convinced that the drug has done some kind of damage that has yet to be determined. There were 2 women online that stated that they looked as though they were pregnant. At the time both were in the process of filing law suits. Both were under the age of 40. I did contact the drug company, they took info, sent out a questionaire and that was it. I believe the only way that I'll make progress is to drastically reduce my calorie intake while at the same time walk long distances and see if I can get it off and keep it off. Bill.
Where did you see those comments? I can understand some abdominal distention while you are on it, due to the fact that anticholinergic meds like spiriva can slow down your gi tract, but that's not weight gain. I have checked 3 references now and none say anything about weight gain, and I've had many patients who used it without weight gain. I'm trying to figure out if there is some other effect it has that could cause weight gain. I can see where symbicort would cause it because its a cortisone. I searched spiriva here in DC, and went back to 2004 and found only one that even suggested weight gain. The responses to that were that it didn't cause it.
I am now looking at information I had run a printout on about 11/2 years ago. There are several people with all the same problem: Weight gain around the middle only with "funny looking bulging" as one woman had mentioned. This info was from a COPD & Emphysema Support Group. Some of these people blame it on "steroids" in the inhhaler but I believe they are wrong.
I have info from a year ago that came from:Spiriva Symptoms and Conditions: Medications.com. Here one woman claims she gained 40 lbs from Spiriva and that she "looks like a pregnant old lady... what a mess. I'm waiting for a class action law suit that should be soon." Even though there was quite a bit of info a couple of years ago I can find nothing more now on Spiriva weight gain.
As I've said, I have gained weight from other medications in the past, like Elavil, but when I discontinued using it the weight came right off without dieting. When I gained weight before it was all over; my legs, hind end, arms and so on. The weight gained here with me, as with the other people, is only on the midsection, and nowhere else! It looks strange indeed. Even my daughter asked a couple of times; "what's wrong with your belly dad". This is a strange one indeed. I think I'm stuck with something I hadn't planned on. Bill
There are no steroids in spiriva so that's not a consideration. I'm going to keep seeking this out, I like to learn things like this, but I want the reason why.
Hello, I'm back. I had a busy day.
I dug up more info on Spiriva weight gain. I found one case of a 61 year old male with COPD on Spiriva. He states: "Extreme weight gain, 55lbs in 11/2 years, all in the middle with bloating." That was from Medications.com almost 2 years ago. There is one more from the same site of a 71 year old male. Here he comments: "... weight gain including an unpleasant distended belly after eating." That part about the disentded belly is very, very true as far as I'm concerned. My belly is always distended outward to the point where it looks strange indeed. That is because it looks way out of proportion with the rest of the body which has gained no weight. With me it is a constant feeling of bloat and fullness, very unfortable at all times. I know this is hard to imagine that an inhaler would have such a side effect. I know it is uncommon, rare, but believe me I know Spiriva has caused this problem.
I have not put any dress clothes on in 2 years because of it. I refuse to accept this as permanent being the reason why I will not buy new clothes with a large size waist size. There must be a solution somewhere. This side effect is not benign as it ruins lives. Many of us do care about out appearance and this is an unacceptable side effect as far as I'm concerned. That is the strangest thing, where it puts the weight. I'll have to keep researching until I can figure it out. I want to know why, lol.
Sounds great. I've looked everywhere within my resources but find nothing. I've searched in vein to find someone, anyone, who has had this problem and found a solution to it. There must be some way to rid one self of such an unsightly condition unless, God forbid, it's permanent damage. If Spiriva is permanently damaging people in this respect then shame on the manufacturer, Boehringer Ingelheim of Ingelheim Germany, for not removing this thing from the market. They have an outlet in this country. Those are the people I contacted by phone. They acted as if they never heard of it, and said they would report it to the FDA. The psychiatrist I see twice a year, a psychopharmachologist, said they are lying, and that they knew exactly what I was talking about! Good luck in your seach. Bill

I hope you can find something. I know that this is a rare side effect that's hardly ever heard of but it is real. There's one thing about those who are affected this way... the weight gain never seems to stop as long as one continues taking it. I've taken different medications that's caused weight gain but in every case the gain leveled off over time in spite of continuing the drug. I'm certain that with this stuff it never stops as long as one continues taking it. I'll never forget a woman who contacted me online 11/2 years ago. She stated that her 62 year old father had taken it for 2 years, and had gained an unbelievable 90 lbs! He finally stopped because of a developing heart condition and diabetes. I wonder what would have happened if he would've continued taking it. I had plenty of questions for her but for some reason I couldn't get back to her. Bill.

Hello, this is Bill, the one who struggled with the Spiriva weight gain. The last time we had contact you'd mentioned that you were curious about this side effect that I've toiled with for the last 2-3 years involving this strange, and yet overlooked side effect. The damn thing ruins lives and should certainly be looked into. Anyway have you taken the time to find anything on this problem? If not then that's alright too. I'll still be here waiting for someone to offer a solution. Incidentally; I've had my testosterone levels checked, once again, and will find out the results next Monday. I still believe this problem goes deeper then that. My levels were normal when this problem started.

No, but I'd contacted the drug company a couple of years ago and they said that they'd contact the FDA and let them know of this side effect. I don't believe one word they said. They knew damn well what I was talking about but let on that it was "news to them." Nonsense. Some of the info I've gotten grom people regarding the weight gain problem goes back far beyond the time I'd talked to this company. It has taken me a long time to come to the realization that things in our society are not being handled as we all expected them to be.
I could tell you a story or two of other medications that have done permanent damage to me but yet nothing has been done about them. I could tell you of the perilous situations that patients are put in by taking some of these toxic conconctions and yet nothing has been brought to the public.
I was ripped apart by an antidepressant several years ago that caused me to, what seemed like, have every muscle in my body pulled apart. The next thing I knew I was in the hospital with a femoral hernia, and emergency surgery. I'd turned diabetic and almost died along the way.
The next day the surgeon came in my room and asked me: "What medications are you on?" My reply was: Zoloft!" His reply was: "Well, you're not going to get that here!" The doctor that prescribed the medication didn't believe what'd happened, never believed one word I said and refused to contact the surgeon whose number I had given him. As I walked out of his office I said: "You're dangerous", and never went back.

I took Spiriva 5 yrs ago, at 37 & gained 80#s in 4 months! My arms, legs, belly, rear & breasts blew up! I called Boehringer Ingelheim Pharmaceuticals, Inc. & spoke to one of their medical reps, approximately 3% of people have had this adverse side effect, so it wasn't enough to make it into their top side effects list. I am a woman & went from 140 to 220, I have had no success at losing the weight, my health actually worsened, needless to say I stopped the Spiriva after 2months & just use my emergency inhaler, which is getting more & more difficult to rely on. I have been with a trainer for over 2yrs, gym for 4yrs, coach softball for 16yrs, saw a nutritionist (no diet issues), was only on pain meds that I had been on for 10 yrs (so I know they weren't the culprit), I have tried South Beach, Atkins, 6wk Body Makeover, anything & everything! Nothing is working! I feel your pain! If someone finds success, let em know, please.
Hello, I just recently quit using spiriva I went from 159 to 228. I had a heart attack July 2013, then open heart surgery September 2013. I quit smoking July 2013 now I have asthma. Was put on spiriva in September 2014 now I have this weight and can't lose it.
There are many people who have complained about extreme weight gain and feeling bloated around the mid section from using Spiriva ! It might not be listed as a side effect by the drug company, but it certainly is happening all around the world. I have experienced it too,
All I need is for someone to come forth and try and give some kind of solution to the problem of weight gain from the inhaler Spiriva. I have gained weight from various medications in the past only to lose it when I discontinued the medication. I have not used Spiriva in 2 years, and yet the weight gained from using it still plagues me in spite of dieting. My belly is all blown out of shape and I look terrible. HELP!
Off Spiriva for 2 years ?? What took it's place?
