I forgot to shake my depo-provera shot. Should I contact my doctor? Will the shot still be effect?

Depo Provera should indeed be shaken vigourously for 1 minute, prior to administration. This is to ensure a uniform suspension (to make sure it's mixed properly).
If the medication is not mixed properly, then it's effectiveness cannot be guaranteed.
