Betrixaban Dosage

Usual Adult Dose for Deep Vein Thrombosis - Prophylaxis

Initial dose: 160 mg orally once
Maintenance dose: 80 mg orally once a day with food
Duration of therapy: 35 to 42 days

Comments:

• Safety and effectiveness have not been established in patients with prosthetic heart valves.

Use: Prophylaxis of venous thromboembolism (VTE) in hospitalized patients with acute illness who are at risk of thromboembolic complications due to restricted mobility or other risk factors.

Renal Dose Adjustments

Severe renal impairment (CrCl 15 to under 30 mL/min):
Initial dose: 80 mg once
Maintenance dose: 40 mg orally once a day
Duration of therapy: 35 to 42 days

Liver Dose Adjustments

Mild liver dysfunction: No adjustment recommended
Moderate to severe liver dysfunction: Not recommended

Dose Adjustments

Concurrent P-gp inhibitor dosing:
Initial dose: 80 mg once
Maintenance dose: 40 mg orally once a day
Duration of therapy: 35 to 42 days

Precautions

US BOXED WARNING:

• SPINAL/EPIDURAL HEMATOMA:
• Epidural or spinal hematomas may occur in patients receiving neuraxial anesthesia or spinal puncture.
• The risk of these events is increased by indwelling catheters or concomitant medical products affecting hemostasis.
• These hematomas may cause long-term or permanent paralysis.
• Consider these risks when scheduling patients for spinal procedures.

Safety and efficacy have not been established in patients younger than 18 years.
