Guaifenesin and hydromorphone has been assigned to pregnancy category C by the FDA. Animal reproduction studies have not been conducted on this combination product. There are no controlled data on this combination product in human pregnancy. Guaifenesin-hydrocodone is only recommended for use during pregnancy when benefit outweighs risk.

There are no data on the excretion of guaifenesin and hydromorphone into human milk. The manufacturer recommends that due to the potential for serious adverse reactions in nursing infants, a decision should be made to discontinue nursing or discontinue guaifenesin and hydromorphone, taking into account the importance of guaifenesin and hydromorphone to the mother.
