Usual Adult Dose for Local Anesthesia
Place 2 soaked cottonoid pledgets (40 mg per pledget) in each nasal cavity for a total dose of 160 mg.
Maximum dose: 160 mg (4 pledgets), or 3 mg/kg, for any one procedure/surgery
Comments: The recommended size of cottonoid pledgets measure 1.3 cm x 4 cm.
Use: Induction of local anesthesia of the mucous membranes when performing diagnostic procedures and surgeries on or through the nasal cavities.
Renal Dose Adjustments
No adjustment recommended.
Liver Dose Adjustments
No adjustment recommended; monitor for adverse reactions such as headache, epistaxis, and clinically-relevant increases in heart rate or blood pressure and do not administer a second dose within 24 hours of the first dose.
Precautions
US BOXED WARNING:
- ABUSE AND DEPENDENCE: Central nervous system (CNS) stimulants, including cocaine hydrochloride, have a high potential for abuse and dependence.
CONTRAINDICATIONS:
- Hypersensitivity to the active component, any of the ingredients, or any other ester-based anesthetic

Administration advice:
- This drug should not be applied to damaged mucosa or open wounds.
- Place 2 pledgets in each nasal cavity against the septum.
- Leave pledgets in place for up to 20 minutes before removing and continuing with the procedure.
- Discard pledgets and dispose of any unused solution in accordance with institutional procedures for Schedule II products.
Storage requirements: Do not freeze.
Reconstitution/preparation techniques: Pour the entire contents of one 4 mL (160 mg) bottle into a small container and soak 4 cottonoid pledgets until the solution is fully absorbed.
